I have seen the same in the past but never been able to identify a set of circumstances that caused it. Simply leaving the room and returning would not be sufficient to clear the problem, but logging off BBO and logging back in would. It may have occurred when the Vugraph operator completed the auction but then undid the final pass. It felt that there was a caching issue in the web client, similar to the problem where the Vugraph operator profile was never updated.
