The key here is to force declarer to play from his hand as many times as possible. And this is true regardless of declarer being 3613 or 3712 or whatever.

You can either switch to low trump now, to make clubs a clean exit from both sides. Or even better play a low diamond to partner's ace, to strip declarer out of diamond escapes. That way partner will have no temptations on ducking one diamond in case declarer has 2 later. But this risks a lot if declarer has ♣Q9x.

If partner has 5 clubs, then declarer has ♣93, if he has 4 then there is no worry, best defence is to play a low club right now to kill dummy's possible entry. And hopefully partner in with ♣Q will realice the need to cash ♦A before playing another club, then we can exit a low trump if its needed.
